My Care Line is a virtual healthcare service that gives Village caregivers direct access to clinical expertise during evenings, overnight hours, weekends, and other moments when a traditional clinic may be closed.
The service was introduced to strengthen continuity of care, reduce unnecessary hospital visits, and support timely decision-making when a resident has a change in condition.
My Care Line is available during the hours when after-hours support matters most.
- Monday through Friday: 6:00 p.m. to 6:00 a.m.
- Saturday and Sunday: 24 hours a day
- Coverage supports access to medical expertise every day of the year
This schedule helps residents and families feel supported when urgent questions come up outside regular office hours.
When a resident experiences a change in condition, Village caregivers can start a My Care Line consultation by phone or video.
Providers can also use the TytoCare device for more detailed virtual assessments. That equipment includes an otoscope, a digital stethoscope, and an exam camera so off-site clinicians can gather better information in real time.
Each encounter is documented in the resident's medical record and shared with the resident's primary care provider.
My Care Line supports a wide range of after-hours concerns, including:
- Falls and post-fall assessments
- Wound checks and skin concerns
- Behavioral or cognitive changes
- Medication questions
- Pain management
- General changes in condition
That support helps caregivers respond with more confidence and helps residents stay in familiar surroundings whenever appropriate.
